Milestone Donor Recognition

Bloodworks Northwest simply wouldn’t exist without our donors. The life-saving impact each and every donor makes on the community is nothing short of profound. Bloodworks proudly celebrates our donors, especially those who reach significant milestones with their life-saving donations.

A Milestone Donor is anyone who has voluntarily given 100, 200, 300 or more red cell, platelet and plasma units.

Donors who reach a milestone will receive a Milestone Donor Recognition Package, which will be mailed to your home address within about two months of your milestone donation date.

The Milestone Donor Recognition Package includes:

  • A personalized letter of thanks from Curt Bailey, President & CEO of Bloodworks Northwest
  • A Certificate of Recognition denoting the milestone reached
  • A Tree of Life – Leaf Request Form

What is the Tree of Life?

The Tree of Life is a decorative wall exhibit, shaped like a tree with metal leaves that display the names of our Milestone Donors as a way of honoring their achievement. A Tree of Life can be found in 11 of our 12 donor centers.

Milestone Donors who wish to have a leaf on one of our Trees of Life can request one at no cost.

To request a Leaf on one of our Trees of Life, you can complete the Leaf Request Form included in the Donor Recognition Package you will receive and mail it to Bloodworks, or you can complete and submit the form below.

You may also download and print the form here. Please email your completed form to [email protected]

PLEASE NOTE that you must be a Milestone Donor in order to request a leaf and that requesting a leaf on the Tree of Life is completely optional. Your leaf should be displayed about two months after your Tree of Life – Leaf Request Form is submitted.

The Bloodworks Donation Center in Eugene does not have a Tree of Life display. When the Lane Blood Center became part of Bloodworks, an established Milestone Donor Recognition Program was already in place. Milestone Donors in and around Lane County receive a card honoring the achievement and the opportunity to have a photograph added to the Milestone Donor Recognition Display in the Bloodworks Donor Center in Eugene. In the future, we will align our Donor Recognition programs.
