Exact match criteria
Pre-Transfusion Samples
The following are criteria for rejection of pre-transfusion samples:
- Not labeled with a minimum of patient Name, identifying number
(MRN), date of collection, time of collection, identity of phlebotomist - Sample is not accompanied by an order requisition with the exact match of Patient name, identifying number (MRN), date of collection, and time of collection. Identity of phlebotomist required, but doesn’t
have to match exactly to label.
The following are criteria for rejection of ABO Confirmatory samples:
- Both bullets above
- Time of collection on confirmation ABO/RH sample and accompanying order requisition is the same as original order
NON Pre-Transfusion Samples
There will be no change in the labeling requirements for specimens sent to BloodworksNW not intended to be utilized for pre-transfusion testing (ex. DAT, Factor 8, HLA, etc.).

Pre-Transfusion Samples Rejection Criteria
The following are criteria for rejection of pre-transfusion samples:
- Not labeled with a minimum of Patient Name, identifying number (MRN), date of collection, time of collection, identity of phlebotomist
- Sample is not accompanied by an order requisition with the exact match of Patient name, identifying number (MRN), date of collection, and time of collection. Identity of phlebotomist required, but doesn’t have to match exactly to label but cannot be discrepant (label can have initials “MJ” and name/signature on order requisition must be in agreement with M and J)
The following are criteria for rejection of ABO Confirmatory samples:
- Both bullets above
- Time of collection on confirmation ABO/RH sample and accompanying order requisition is the same as original order
Rejection Policy
When unacceptable samples are received, BloodworksNW laboratory personnel will notify the ordering facility of the rejection and the need for recollection.
Rejection Criteria for Samples Not Intended for Pre-Transfusion Testing
There are no changes in the rejection criteria for specimens sent to BloodworksNW not intended to be utilized for pre-transfusion testing.
